Ureteral Access Sheath-Y Type

A Y-Type Ureteral Access Sheath (UAS) is a specialized urological instrument designed to facilitate Retrograde Intrarenal Surgery (RIRS), primarily for the treatment of kidney stones. Its defining characteristic is a Y-shaped connector at its proximal end, which provides two independent pathways: one for the primary surgical scope/instruments and a dedicated second one for continuous irrigation or the passage of auxiliary tools. This design offers superior control over fluid dynamics within the kidney compared to standard sheaths, enhancing both safety and efficiency.

 

    Product Features

    ● The hub splits into a main working port and a separate side port. The side port is dedicated to continuous inflow irrigation and is connected to the space between the sheath and the scope.
    ● The side port often has a hemostatic valve, allowing a guidewire or laser fiber to be inserted alongside the scope without removing it.
    ● The sheath is designed from advanced polymers to resist kinking and compression in the tortuous ureter, ensuring uninterrupted flow and scope movement.
    ● Many sheaths have a coating that becomes extremely slippery when wet. This drastically reduces friction, making insertion into the ureter smoother and more atraumatic.

    INSTRUCTION FOR USE

    ● Inspect the device. Flush both the main channel and the Y-side port with saline to activate the hydrophilic coating and ensure patency. Connect irrigation tubing to the Y-port.
    ● Place a guidewire up the ureter into the kidney.
    ● Under continuous fluoroscopy, advance the pre-assembled sheath and dilator over the guidewire.
    ● Use gentle pressure. STOP if significant resistance is met to avoid ureteral injury.
    ● Position the distal tip in the proximal ureter.
    ● Remove the inner dilator, leaving the outer sheath in place.
    ● Begin continuous irrigation through the Y-side port. This provides high-flow fluid to maintain a clear view during the procedure. 
    ● The Y-port can also be used to insert instruments (e.g., laser fiber, guidewire) alongside the scope without removing the scope.  
    ● Insert the flexible ureteroscope through the main working port to perform the surgery (e.g., laser lithotripsy).  
    ● Withdraw the scope. Remove the sheath over a guidewire using a slow, twisting motion under fluoroscopy.
